OVERVIEW

We realize that individuals want to reach their full potential with the support of their friends, family and community. In some cases, just one service could be beneficial for someone. In other cases, several may be needed for appropriate support. Thrive customizes the services to suit each individual's need so they may live an independent and full life. Residential

Thrive provides personalized supports for adults with developmental disability in their own home and in the community. As a provider, we recognize the important choice of where you live, the connections you make, and the responsibility of being a good neighbor. We advocate for the right for each person we serve to choose the way they live to achieve their own goals with the support of a team to provide security and knowledge to gain the skills to advance independence. 

Choice is a vital component to preserve the dignity and personality of each individual so they may exercise their rights as a person with developmental disabilities.

Our supports aim to achieve independence, connectedness, and achievement for the most important person on the team: the individual. 

On your team will be the staff providing services tailored to each individual to encourage independence while receiving care and becoming a intricate part of one another's lives. 

This includes assistance with daily living tasks such as cooking, cleaning, personal hygiene, shopping, attending medical appointments, budgeting, and other functions of living in your own home. 

Everyone enjoys the feeling of belonging and the best way to get out and make connections is to be involved with your community. This includes planned activities the individual chooses to attend with staff support. 

At times, it may be necessary for a primary caregiver for an individual to take time away to engage in self-care. During that time, it is important to entrust your loved one to someone you trust. Thrive provides respite services to allow caregivers to disengage and recharge and individuals the opportunity to experience a new environment and make friends.

Vocational

Here at Thrive, we understand that everyone has an innate desire to contribute to our communities in a meaningful way. The way we contribute will look different to each individual. To promote choice and independence, Thrive provides assessment, placement and job coaching to individuals that already have sufficient skills to work in community employment. 

For those that may need to develop their skill base, education is available in via our Job Readiness program that will assist individuals in strengthening or developing the necessary knowledge, and professional and social skills that will help them succeed their preferred occupation.  

Those that seek to increase their independence with obtaining more life skills are able to engage in activities that teach cooking, cleaning, safety skills and more.

All individuals can engage with these skills while enjoying adult day services that will provide opportunities for volunteer work and socialization with peers and community members. 

Housing

We recognize that the Pandemic has made finding and obtaining housing difficult for many. Individuals with disabilities have been impacted due to the shortage of accessible, affordable housing. Thrive is currently working with Shared Living providers, landlords and various city governments to obtain homes for individuals with disabilities to help those that would like to live alone, with roommates, or with a Shared Living provider to help increase choices for individuals and alleviate housing insecurity. 

Benefit Management

While some individuals have family members or guardians that will help them manage and renew their benefits, others may require the help of a third-party to help. We are able to help with Medicaid/Medicare, Social Security, budgeting, and assuming representative payee status for individuals when needed in some cases. 

Transportation

Getting around town can be a hassle when transportation must be arranged for an individual with mobility needs. Thrive provides non-medical transportation to individuals with IO Waivers and Level 1 Waivers to provide more access to the community.
